Case presentation

A 40-year-old female patient with complaint of heavy menstrual bleeding.

Medical history

  • No history of renal or hepatic impairment.
  • No coagulation or fibrinolytic disorder.
  • No previous history of thromboembolism, peptic ulceration and inflammatory bowel disease.

Diagnostic Tests:

Following diagnostic tests were conducted
  • Transvaginal ultrasound (TVS).
  • Paps smear.
  • Complete blood count (CBC).
Out-comes of diagnostic test
  • TVS and Paps smear was found to be normal.
  • Complete blood count elicited hemoglobin level of 9 g/dl.

Treatment:

Combination of 500 mg Tranexamic acid and 250 mg Mefenamic acid was administered 2 tablets administered immediately followed by 1 tablet every 8 hrs. for 5 days beginning from day 1 of menstrual cycle.

Follow up after 3 months:

  • The menstrual blood loss on an average was reduced.
  • Bleeding was scanty in last two days and lasted for 2-3 days.
  • 1.5-2 gm% rise in the hemoglobin levels was seen.
  • 55-60% decrease in the use of sanitary napkins was observed.

Conclusion:

Mean menstrual blood loss was reduced after initiation of treatment with combination of Tranexamic acid and Mefenamic acid which also helped in reducing dysmenorrhea.
